I did try the form and get this error on form submit

Hi Kenny, 

That error means the email credentials weren't accepted.

Can you confirm the SMTP details under Admin > Email Settings are correct? 

You could also try them in an external email program or ask your webhost to confirm what the latest credentials you should be using are.

Thanks for the suggestion, what would the most basic version of that do?  Generate email forms?
