I'm having same issue, and none of the solutions listed are working. When I upload image in new installation in CMS builder, I get message " No matching uploads were found!" When I turn off flash uploads, error is "MySQL Error: Unknown column 'thumbFilePath2' in 'field list'"
I believe all permissions are correct, and all files actually do upload and show via ftp, just not in CMSB.
I also just noticed that the user accounts are missing. Can't add users.... are these related, and is there a fix?

Have you made any changes to CMS Builder recently, for example upgrading to a later version or moving servers?
