Website membership checkboxes and image upload

Hi Jason,
Thank you for checking my file.
I've tried everything all over from the beginning and everything works now except for the single checkbox and uploads.
For checkbox, I changed it to a radio button and it looks good now so maybe I will try the checkbox next time.
For uploads, I made a new page uploadImages.php to make thing more simple for me. I only changed my table name, field name and the die("Invalid recordNum"); ... part from your reply.
It still says "Invalid recordNum"
I have the correct record num at the top of the form.
