Image uploads fine but no preview on insert window

I followed the image upload tutorial - well, I think I followed it.
Images seem to upload to my server alright, although I see the warning error sign in the bottom left corner of the upload window. Not sure what that means as I can't find a way to uncover the error it's indicating
The big problem I see right now is I can't see a preview of the image in the insert window, and when I try to insert the image, it doesn't appear - just an image placeholder.
Troubleshooting hints welcome !

Hmm, sounds like it's not quite setup all the way. Without taking a look, it's hard to say where the problem might be. If you'd like to fill out a support request, we can probably track that down for you. :)

That would be great.
I entered a support request - but forgot one important thing. Would you please let me know what I goofed up?
I expect to be installing this on a number of other sites and don't want to be pestering you for help each time !
