Image upload form for new item not working on new or existing items

Image management has become an issue in Listings Manager.
When adding a new item, we can browse the hard drive, select the image, click upload and the results form tells us the image is missing. The image name does not appear in any of the select fields when we try to add to the item listing.
When managing an existing item that has images (and they display properly on the website), we are unable to manage by adding/editing or deleting the images because they too do not appear in the selection drop down fields.
We've tried this in IE, Firefox and Chrome. There are no space limitations set on the server and permissions for the image folders are set to 777.
Can someone assist please? We'd appreciate it very much.
