Hi,
I need to create a 'contact us' form within an AM page, can anyone help please?.

Hi Michael,

If you'd like to set up a Contact form that will allow people to send you an email on your site you should be able to track down an email script that will do this for you.

You can have a look at one script I've worked with in the past here:

There are all sorts of mail scripts so if you're able to track one down you should be able to get this into the content of an Article Manager article or put this right on the template file for a specific category.
